Be ready to fill out and upload a Minnesota ST19 Form, which requires your Minnesota Tax ID OR Social Security Number

Artists cannot change the title or sale price of their artwork between the submission and delivery of artwork.Entries previously submitted to Primavera will not be accepted.The fee charged is an application fee and is non-refundable whether your entry(s) are accepted into the show or not.2D work must be dry, framed (or gallery wrapped with finished edges for works on canvas) and securely wired for hanging. NO SAWTOOTH HANGERS, PLEASE.Artists are required to pay the Plymouth Arts Council 30% of the purchase price for artwork displayed and sold during Primavera. You will be invoiced.The maximum size of each entry is 24"x36" for 2-dimensional art or a comparable size for 3-dimensional art.Artwork must not exceed 20 pounds.I verify that I have personally created the listed work to be shown. I will not hold the City of Plymouth, Plymouth Arts Council, or its volunteers responsible for any damages, theft, or loss of my work while it is in their possession. I give the City of Plymouth and the Plymouth Arts Council the rights to use my artwork for promotional purposes as related to Primavera.All artists must include a completed Minnesota ST19 form with their application.

Once you've download the form, fill it out completely, save it to your computer or device, and then upload a PDF version of the completed form in the box below. You will be required to enter your Minnesota Business Tax ID or your social security number. If you do not have a Minnesota Business Tax ID and prefer not to use your social security number, you can sign up for a MN Tax ID at the department of revenue website (web address below).
