We are thrilled that you will be attending the Dental Wisdom Conference February 5-7, 2026! When registering, you indicated that you would like to add on your spouse to the Thursday and Friday lunches, covering their cost (please note that lunches for spouses on Thursday and Friday are not included in the registration fee). The cost is $110 per person per day ($220 if joining both days) if you would like them to join. There is no obligation for them to join based on your indication during registration -your spouse may choose to make alternative arrangements - but please complete this form and make payment if you would like them to join; we are essentially passing these costs directly to the caterer. For planning purposes, it would be helpful if you completed this as soon as possible. Please note that this is a non-refundable selection.
