Official Entry Form ~ CPE Speedway Trial

CERTIFICATION OF AGREEMENT UPON ENTRY OF DOG: I certify that I am the actual owner of the dog, or that I am the duly authorized agent of the actual owner whose name I have entered above. In consideration of the acceptance of this entry, I agree to be bound by the following:

DISCLAIMER AND INDEMNITY 

I/we acknowledge that I am/we are familiar with the current rules applying to Canine Performance Events (CPE) Trials, including all terms and conditions contained in this Event Premium, and agree to abide by all rules and T&C. I/we agree that the Host Club holding this trial has the right to refuse entry for cause which the Host Club, in its sole judgment, must deem to be sufficient. Upon acceptance of this entry, I/we agree to hold (a) Canine Performance Events, Inc., (b) City Sniffers Dog Sports Training (the Host Club), (c) Rex's Place (the owner of the premises), (d) any sponsors of this event, and each of their members, directors, officers, agents, Trial Secretary, Trial Chairperson, Trial Committee and employees harmless from any claim or loss or injury or expenses (including attorney fees) which may be alleged to have been caused directly or indirectly to any person or thing arising out of or in consequence of my/our participation in the SpeedWay trial or the act of my/our dog, howsoever such injuries or damage may be caused and whether or not the same may have been caused by negligence of the aforementioned parties or any of their employees or agents, or any other persons. I/we further agree to hold the aforementioned parties harmless from any claim or loss of this dog by disappearance, theft, death or otherwise, and from any claim for damage or injury to the dog, whether any claim be caused or alleged to be caused by the negligence of the Host Club or any of the parties aforementioned, or by the negligence of any other person, or any other cause or causes.

I/we testify that the dog entered is healthy and vaccinated or titered. I/we certify that the dog entered is not dangerous to any person or other dog. 

Changes to trial results will not be made once the trial has been completed. Handlers are responsible for the accuracy of information in their confirmation from the Trial Secretary (i.e., dog’s information), prior to running in a trial, and for reporting any discrepancies of posted results prior to leaving the trial site each day. Includes if there was a correction to a scribe sheet then the handler must verify that the results were corrected as well. Labels are not official results.

MEDIA RELEASE: I hereby authorize club officials and/or club-appointed videographer and/or photographer to use, reproduce, and/or publish photographs and/or video that may pertain to me, including my image, likeness and/or voice without compensation. I understand that this material may be used in various publications, public affairs releases, recruitment materials, on the City Sniffers Dog Sports Training, Rex's Place, and/or Canine Performance Events websites or social media accounts, or for other related endeavors. This authorization is continuous and may only be withdrawn by my specific rescission thereof. Consequently, Canine Performance Events, City Sniffers Dog Sports Training, and Rex's Place may publish materials, use my name, photograph, and/or make reference to me in any manner that the Corporation or event sponsor deems appropriate in order to promote/publicize service opportunities.

By signing below, I/we indicate that I/we have read and agree to the AGREEMENT, DISCLAIMER & INDEMNITY, AND RELEASE obligations above and agree to the conditions.

Facility Policies

*Designated potty areas are the grassy areas on the streets around the building; there are also multiple entry points to parks/walking paths within a one-two block radius.
*You MUST pick up after your dogs -- dog waste buckets are posted outside access doors.
*Dogs must be on leash when entering the building; no socializing with other dogs.
*Dogs may only be off leash during their turns in the fenced rings.
*Dogs must go in crates between runs; you may bring your own travel crate or crate out of your car.
*To prevent disruptive/excessive barking: handlers are encouraged to bring Kongs, lick mats or snuffle mats to keep dogs busy during their downtime.

Trial Registration Policies

Entries are accepted online via this form (strongly preferred) or by mail only – no drop-off, phone, OR email entries. Dogs must have a CPE member ID number: register here: https://cpe.dog/members. 

Exhibitors are responsible for complete, accurate and legible information on entry forms; illegible or incomplete entries will not be entered in the trial. Always check your confirmation!

Entry Limits: This entry form will be closed (or options modified) when run limits are reached. If the form is allowing your entry, you will have a spot in the trial!

Withdrawals & Refunds: Competitors who withdraw prior to the pre-entry closing date of Wednesday, February 11, will receive a refund of all entry fees, minus any non-refundable credit card transaction fees (if applicable). There is no refund of entries withdrawn after the closing date, regardless of reason. 

Entry fees will not be refunded in the event that a dog is absent, disqualified, excused, or barred from competition by action of the Trial Committee. It is expressly understood that exhibitors alone are responsible for the behavior of their dog(s) and/or children. Any exhibitor whose dog(s) and/or children create unnecessary disturbances or repeatedly engage in unsafe or disruptive behavior may at the discretion of the Trial Committee, be asked to leave the trial site. If an exhibitor is asked to leave the trial site, then the exhibitor will not be refunded any fees that have been paid.

Entry fees will not be refunded if the trial cannot open or be completed by reason of riots, civil disturbances, fire, an act of God, public emergency, act of a public enemy, or any other cause beyond the control of the organizing committee.
