Camper Health Information

At Hidden Bee Farm and Hidden Bee Learning, we collect health information for each camper to help ensure a safe and supportive environment during all camp activities.We are not able to administer medication. However, it is important for us to be aware of any medical conditions, allergies, or medications your child may take so we can respond appropriately in the event of an emergency.Please complete each section as accurately as possible. Is your child currently taking any medications that we should be aware of? While Hidden Bee Farm and Hidden Bee Learning staff are not able to administer medication, it is important for us to be aware of any prescription or over-the-counter medications your child takes in case of an emergency.
Acknowledgment of Program Guidelines and Release Forms
By submitting this form, I confirm that I am the parent or legal guardian of the camper listed above and that I am at least 18 years of age. I acknowledge that I have read and agree to all program policies, liability waivers, and related forms.

In the event of a medical emergency or need for general medical care, I authorize qualified personnel to provide necessary treatment for my child. I understand that my child will only be released to the individuals listed on this registration form. I further give permission for my child to attend camp and participate in all scheduled activities.
I authorize Hidden Bee Farm / Hidden Bee Learning to use photos and/or videos of my child for promotional purposes unless I opt out on the registration form.

Camp tuition is charged per session and listed with each camp option
A $50 non-refundable deposit per child is required to secure a spot
