PLEASE READ THE BELOW INFO BEFORE FILLING OUT THIS FORM.
1. We will be using the contact information (including address, for any awards you may earn) that you currently have on file under the dog you enter below. The email, mailing address, etc, that you see listed in your NADAC Portal, is what we have on file. Visit the NADAC Portal to check or update your info, asap, if you have not recently.
2. Because of the info listed above, it is crucial that you fill in your dog's NADAC number correctly, as well as your name.
3. It is equally as crucial, for our scoring software import, that you list your team name EXACTLY the same as your teammates. If your teammates list a team of "Flying Fidos" and you list a team of "Flying Dogs", it will cause a mess.
4. Each member of your team(s) needs to fill out this form separately and submit.
5. The scoring submission form, for each round, will be on the Team Series page of the NADAC Website. Do NOT submit your Virtual Team runs to the NADAC VT System.
6. Please read the Team Series Rules before entering. You can find the Rule's document on the NADAC Website. Click on the Train and Play tab and then select "Team Series".
