SDDT's Valentines Day Parent's Night Out
Valentines Day is fast approaching and we know how important it is for parents to have a night out every once in a while. So SDDT invites children ages 3 and up on Friday February 13th from 6:30pm-9:00pm to our Princess & the Frog night! During this event we will make a Valentine craft, create a short dance, eat a snack & watch a movie. The cost for 1 child is $35 and each additional sibling is $15. Please RSVP by February 5th via the form below. For this event we are only accepting online registrations and online payments! You wont want to miss this night!

I, the undersigned, give permission for my child(ren) to participate in Parent's Night Out with South Dayton Dance Theatre. I recognize the possibility of physical injury or illness to the child(ren) associated with taking part in this activity. Upon signing this waiver, release and forever discharge any and all rights and claims for damages, which may arise now or in the future against SDDT and its owners, employees and/or other assigned representatives or volunteers from any and all liability and for any and all damages and/or injuries/illnesses which may be sustained or suffered by the student while participating at or for SDDT. Furthermore, in the event SDDT is unable to contact the authorized person on this form, I hereby give my consent for emergency medical care. Transportation to the hospital will be at the discretion of the Emergency Technicians on site.

I fully understand that all above materials recorded may be used now and in the future by the South Dayton School of Dance and/or the South Dayton Dance Theatre.
