Sunshine and Smiles Summer Camp Registration

Sunshine and Smiles Summer Camp does not offer refunds or make-ups for sickness or absence. Additionally, there are no refunds for campers who are removed from camp due to misbehavior or for camper sessions not completed. In an event or circumstance beyond the control of Sunshine and Smiles Summer Camp, no refunds will be issued. However, in cases of serious illness or injury, compensation will be determined at the sole discretion of Sunshine and Smiles Summer Camp upon receipt of a doctor's note.

All registrations require a $300 deposit upon sign-up, which is non-refundable. Remaining balances are to be charged May 15th unless enrolled in Flex Pay.

Our Flex Pay plan at Sunshine and Smiles Summer Camp allows parents to make weekly payments two weeks prior to the week of camp. For example, for Session 1 starting on June 1st, the first payment would be due on May 18th. A $300 deposit is required upon registration, which will be applied towards the final payment based on the child's registration dates. The exact draft dates are listed in the chart below and will be based on individual campers registered sessions.

In consideration of being permitted to utilize the facilities, services, and programs of Sunshine and Smiles Summer Camp (or for my children to so participate) for any purpose, including, but not limited to observation or use of facilities or equipment, or participation in any off-site program affiliated with Sunshine and Smiles Summer Camp, I, the undersigned, for myself, such participating children, and any personal representatives, heirs, and next of kin, hereby acknowledge, agree, and represent that I have, or immediately upon entering or participating will, inspect and carefully consider such premises and facilities or the affiliated program. It is further warranted that such entry into Sunshine and Smiles Summer Camp for observation or use of any facilities or equipment or participation in such affiliated program constitutes an acknowledgment that such premises and all facilities and equipment thereon and such affiliated program have been inspected and carefully considered, and that I find and accept them as being safe and reasonably suited for the purpose of such observation, use, or participation by myself and such children. In further consideration of being permitted to enter Sunshine and Smiles Summer Camp for any purpose, including, but not limited to observation or use of facilities or equipment, or participation in any off-site program affiliated with Sunshine and Smiles Summer Camp, I hereby agree to the following: 1. I, on behalf of myself and such children, hereby release, waive, discharge, and covenant not to sue Sunshine and Smiles Summer Camp and all branches thereof, its directors, officers, employees, and agents (hereinafter referred to as "releasees") from all liability to me or such children and all my personal representatives, assigns, heirs, and next of kin for any loss or damage, and any claim or demands therefore on account of injury to the person or property or resulting in the death of me or such children whether caused by the negligence of the releases or otherwise while I or such children are in, upon, or about the premises or any facilities or equipment therein or participating in any program affiliated with Sunshine and Smiles Summer Camp. 2. I hereby agree to indemnify and save and hold harmless the releasees and each of them from any loss, liability, damage, or cost they may incur due to the presence of me or such children in, upon, or about the Sunshine and Smiles Summer Camp premises or in any way observing or using any facilities or equipment of Sunshine and Smiles Summer Camp or participating in any program affiliated with Sunshine and Smiles Summer Camp whether caused by the negligence of the releasees or otherwise. 3. I understand that certain on-site or off-site activities, field trips, programs, or services affiliated with Sunshine and Smiles Summer Camp may require additional waivers, acknowledgments, or releases from third-party venues or activity providers. I consent to my child’s participation in such activities and acknowledge that Sunshine and Smiles Summer Camp may complete, acknowledge, or accept any required activity-related forms or waivers as part of routine participation in these programs. I understand that these activities are offered as part of the camp experience, are voluntary, and are subject to the terms and conditions of the activity provider. 4. I hereby assume full responsibility for and risk of bodily injury, death, or property damage to me or such children due to the negligence of releases or otherwise while in, about, or upon the premises of Sunshine and Smiles Summer Camp and/or while using the premises or any facilities or equipment thereon or participating in any program affiliated with Sunshine and Smiles Summer Camp. I further expressly agree that the foregoing Releases, Waiver of Liability, and Indemnity Agreement are intended to be as broad and inclusive as is permitted by the law of the State of Florida and that if any portion thereof is held invalid, it is agreed that the balance shall, not withstanding, continue in full legal force and effect. I have read and voluntarily sign the Releases and Waiver of Liability and Indemnity Agreement, and further agree that no oral representations, statements, or inducements apart from the foregoing written agreement have been made.

I understand that in case of an accident or serious illness requiring immediate medical attention for my child, Sunshine and Smiles Summer Camp will contact me. If I cannot be reached, emergency rescue will be called. If my child is unable to remain at camp due to an accident or illness not requiring immediate treatment, the camp will contact me to arrange transportation for my child.I understand that students with communicable diseases or infestations will not be permitted to attend camp and will be excluded until they are shown to be free of infection or infestation. The camp clinic is staffed with volunteers and is used for minor accidents or illnesses, with a time limit of thirty minutes. I acknowledge that it is my responsibility to notify the camp in writing of any changes to the information provided on this form.

Campers are provided with one T-shirt at no cost, additional shirts can be purchased at $10 per shirt. Reminder: Tshirts are required to be worn daily. Order will be charged to credit card on file.
