TERMS AND CONDITIONS
I, the undersigned, give permission for my child to participate in camp at the Maplewood Area Historical Society - Bruentrup Heritage Farm or Eder Schoolhouse for the days they attend.
I fully understand and authorize my child may be photographed, video and/or audio recorded for MAHS and program promotion and record keeping.
I fully understand that my child is responsible for their possessions including daily lunch.
I understand that in the event of an emergency affecting my child while participating in camp at the Bruentrup Heritage Farm, a designated employee of MAHS will attempt to contact me and inform me as soon as possible. In the event I cannot be reached, I hereby give permission for my child to be treated or hospitalized by a licensed physician or hospital selected by MAHS.
I understand that my child must must follow safety and behavior guidelines, which include no inappropriate language, inappropriate physical contact, and other safety and behavior policies developed and implemented by MAHS staff. Exclusion from certain activities or nonrefundable expulsion from camp may result if my child does not follow the safety and behavior policies.

Refunds for enrollment cancellations (minus a $100 deposit) may be issued up to three weeks before the start date of camp. Refunds are not available for cancellations with less than three weeks’ notice, as we pay for staff and materials up-front. If you have any questions, please contact us at info@MaplewoodMuseum.org.
