  • Welcome to Sorted Planning. 

    This forms helps me understand what brings you to Advance Care Planning and how I can support you during this process. It takes about 10-15 minutes to complete. 

    Your responses are confidential and used only to prepare for our work together. Most questions are required, but some are optional. There are no right or wrong answers. 

    You can save your progress and return at any time. 

    What happens after you submit:

    • I'll review your form within 2 business days. 
    • I'll email you to schedule Session 1 (typically within 1-2 weeks)
    • You'll receive your Pre-Session Guide to help you prepare
    • Sessions are held via Google Meet or in-person, and last approximately 90 minutes.

  • Goals for Advance Care Planning (ACP)

  • ACP helps you think about and share what matters to you in case you become
    seriously ill or injured and can't speak for yourself.

    If it's important to you that your wishes are respected when you can't communicate them yourself, ACP is a valuable tool—no matter your current health.

  • These scales help me understand how to pace our conversations. There's no 'right' answer—everyone starts from a different place, and that's completely okay.

  • Sorted Planning Service Disclaimer & Agreement

    This Agreement outlines what Sorted Planning offers, what you can expect, and the boundaries of the Advance Care Planning service. Please review carefully.
  • 1. Purpose of Services

    Sorted Planning provides Advance Care Planning (ACP) coaching, education, and document preparation. This includes:

    • guided conversations about values, priorities, and goals
    • plain-language education about ACP and Substitute Decision-Makers (SDMs) in Ontario
    • support in identifying your SDM(s)
    • preparation of a personalized ACP Summary Document.

    These services are not:

    • medical advice
    • legal advice
    • mental health counselling or therapy
    • crisis support.

    I do not:

    • guarantee any particular medical outcome or that all wishes will be followed exactly
    • store or maintain your medical records. 

    While Advance Care Planning helps guide future decisions, it may not always be possible for healthcare providers to follow every wish exactly due to medical circumstances or legal requirements. The purpose of ACP is to guide decisions as closely as possible to your values and preferences.

  • 2. Scope of Work

    The ACP service includes:

    • Session 1 (values, roles, SDMs, foundations)
    • Session 2 (review guides, discuss scenarios, clarify wishes)
    • Access to the Medical Interventions Guide and Scenario Workbook
    • Preparation and delivery of a personalized ACP Summary Document
    • 30-day follow-up call
    • Optional annual review reminders


    Additional meetings or custom documents may be billed separately.

  • 3. Client Responsibilities

    Your rights:

    • All participation is voluntary
    • You may stop or take a break at any time
    • You may decline to answer any question
    • Our conversations are confidential within the limits of the law.

    Your responsibilities:

    • Attend scheduled sessions or provide 24-hour notice to reschedule
    • Complete required forms and reflections to the best of your ability
    • Provide accurate information about preferences, values, and health context
    • Communicate any accessibility needs 
    • Seek medical, legal, or mental health support from qualified professionals when needed
    • Share your completed ACP Summary Document with your chosen decision-makers and healthcare providers.
  • 4. Capacity and Appropriateness of Services

    Sorted Planning presumes that all clients have the capacity to engage in Advance Care Planning. If during our work together I have concerns about your ability to participate meaningfully, I will pause our sessions and recommend you speak with a healthcare provider before continuing.

    ACP is not appropriate for individuals experiencing:

    • Active psychosis or severe mental health crisis
    • Moderate to severe dementia or cognitive impairment
    • Delirium or acute confusion
    • Situations involving coercion or undue influence

    If any of these circumstances arise, I will work with you to connect you with appropriate support and will provide a full refund.

  • 5. Payment Terms

    Your Investment:

    • Virtual (standard service): $450
    • In-person (Hamilton only): $530

    Payment is collected in two stages:

    First payment (50%) — due today, on the next page:

    • $225 (virtual) or $265 (in-person)
    • Covers Session 1, Session 2, and all preparation work between sessions
    • Non-refundable after Session 1 is completed

    Second payment (50%) — due after Session 2:

    • $225 (virtual) or $265 (in-person)
    • Collected via Post Meeting 2 Form before your ACP Summary is drafted
    • Non-refundable once drafting begins

    Rescheduling & Cancellation:

    • Rescheduling requires 24 hours' notice where possible
    • Cancellations with less than 24 hours' notice may result in the session being forfeited
    • You are welcome to pause the process at any time and return later with no penalty
    • If you complete Session 1 but choose not to proceed to Session 2, you will not be charged the second payment

    Refunds:

    • Before Session 1: Full refund minus $50 administrative fee
    • After Session 1 is completed: First payment becomes non-refundable (session-based work has been delivered)
    • You will not be charged the second payment unless you choose to proceed with the ACP Summary Document
    • Once the second payment is made and drafting begins, the second payment is non-refundable
  • 6. Privacy & Confidentiality

    I take your privacy seriously. Information you share through forms and sessions is kept confidential and stored securely using reputable digital tools. 

    With your consent, sessions may be audio recorded to support accurate note-taking and the preparation of your Advance Care Planning Summary. Recordings are reviewed only by me, stored securely, and not shared with anyone else. You may decline recording or withdraw consent at any tim

    Your information is used only to support your advance care planning and is not shared without your permission, except where required by law.

    Records are retained for up to 7 years from your last session. You may request access to or deletion of your information at any time.

  • 7. Communication Boundaries

    Between sessions, clients may contact Sorted Planning via email for:

    • clarifications
    • accessibility needs
    • brief check-ins

    Lengthy discussions may require booking an additional session.

    • guided conversations (Meetings 1 and 2)
    • and the creation of your personalized ACP Summary.

    Your Investment: $450 (virtual) or $530 (in-person, Hamilton only)

    Payment is collected in two stages:

    Today's payment (50%):

    • $225 (virtual) or $265 (in-person)
    • Secures your Session 1 and Session 2 bookings
    • Covers all preparation work and workbooks between sessions
    • Non-refundable after Session 1 is completed

    Second payment (50%):

    • $225 (virtual) or $265 (in-person)
    • Due after Session 2, collected via Post Meeting 2 Form
    • Required before I begin drafting your ACP Summary Document
    • Covers document creation, 30-day follow-up, and annual review reminders
    • Non-refundable once drafting begins

    You're welcome to pause the process at any time. If you don't proceed to Session 2, you won't be charged the second payment.
