Duration: 10 week soccer program with 1 practice held once a week.

Fee: $130/child

By signing below I understand that payment is due in full prior to my child’s participation. I agree to submit $130 per child and acknowledge that fees are non-refundable except in cases determined by program administration.

In the case I am enrolled in a payment plan, I agree to pay $45.00 now, a second payment of $45.00 the following week and then a third and final payment the third week. I acknowledge that fees are included in the payment plan and are non-refundable except in cases determined by program administration.

I understand that participation in Homeschool Soccer Academy involves physical activity and carries inherent risks. By signing below, I release Homeschool Soccer Academy, coaches, volunteers and program organizers from any liability for injuries, accidents, or damages that may occur during practices, games, or related activities.

I acknowledge that I am responsible for my child’s medical expenses arising from participation.

In the event of an emergency where I cannot be reached, I authorize the coaches, program organizers, or designated volunteers to seek medical care for my child. I agree to assume all costs associated with such care.

By signing below I acknowledge that my child may be captured in photographs and videos. I grant Homeschool Soccer Academy permission to take photos or videos of my child during program activities for promotional purposes, including social media, flyers, and program updates.

Nondiscrimination Policy:
Homeschool Soccer Academy admits participants of any race, color, national origin, sex, religion, and disability and does not discriminate on the basis of any legally protected status in the administration of its programs, activities, or policies.

Right to Refuse Service:
Homeschool Soccer Academy reserves the right to refuse service or remove any participant from the program at its sole discretion for reasons including, but not limited to: safety concerns, disruptive or inappropriate behavior, failure to comply with program rules, lack of payment, or any conduct that interferes with the safety, well-being, or integrity of the program. This right will be exercised in a fair and consistent manner and never on the basis of a protected characteristic.
