We are excited to have you come to get a custom tattoo, piercing or jewellery purchase and are just a few steps away from finalizing your purchase. With the holiday season upon us, we would like to share a gift with you. For every $100 that you purchase we will give you a $25 bonus Gift Card. The $25 bonus gift card will be given on proof of receipt, in store and will expire in three months from the time of purchase on March 31st.
To finalize your holiday gift certificate, we require you to fill out the following information to keep your gift certificate purchase on file for our records.
I, understand that by filling out this form I am purchasing a gift certificate from Pushing Inc. Tattoo Emporium.
I Acknowledge that the gift certificate is non refundable and may only be used at Pushing Inc. Tattoo Emporium . Gift certificates are NOT redeemable for cash. Gift Certificate can NOT be used as/for a deposit. Must abide by the rules and regulations of Pushing Inc. Tattoo Emporium.
I acknowledge that for every $100 Gift Card purchased a $25 bonus Gift Card will be gifted by Pushing Inc. Tattoo Emporium. The $25 bonus gift card will be given on proof of receipt in store and will expire on March 31st of the consecutive year.
For example, purchase date:
1) December 1, XXXX, $25 bonus Gift Card will expire on March 31st, XXXX.
2) December 20, XXXX, $25 bonus Gift Card will expire on March 31st, XXXX.
If the artist of choice is booking outside of the expiry date the $25 bonus Gift Cards will be honored. In this situation, if the client cancels twice the $25 bonus Gift Cards will be null and void.
