PD Day Waiver for Lootie Camp
1. Acknowledgment & Assumption of Risk
I, the undersigned parent/legal guardian, understand that participation in the PD Day Camp (“the Program”) involves normal physical activities including, but not limited to, crafts, games, outdoor play, movement activities, and the use of various materials, tools, and equipment.
I acknowledge that participation may involve risks of minor injury (such as scrapes, bumps, allergic reactions) and rare but possible serious injury.
I voluntarily assume all risks associated with my child’s participation.
2. Health & Medical Authorization
I confirm that my child is in good health and able to participate in all activities.
I agree to disclose all medical conditions, allergies, medications, or additional needs prior to attendance.
In the event of an accident, illness, or emergency, I authorize the staff of Lootie Patootie to administer basic first aid and to seek medical treatment as necessary. I understand that every effort will be made to contact me immediately.
I agree to be financially responsible for any medical treatment required.
3. Allergies & Food Handling
I understand that Lootie Patootie is a candy and party-based environment and may contain gluten, nuts, dairy, soy, and other allergens in the building. While precautions are taken, a nut-free or allergen-free environment cannot be guaranteed.
I accept full responsibility for communicating all allergy information and supplying any required medication (e.g., EpiPen).
4. Personal Items
Lootie Patootie is not responsible for lost, stolen, or damaged personal items brought to the Program (including toys, devices, clothing, or accessories).
5. Drop-Off, Pick-Up & Supervision
I understand that:
I must sign my child in and out of the Program.
Only authorized individuals may pick up my child.
Late pick-up fees may apply.
I acknowledge that staff will provide supervision according to provincial standards and program ratios.
6. Behaviour & Dismissal
I agree that my child must follow all safety instructions and behave respectfully toward staff and other children. Lootie Patootie reserves the right to remove a participant if behaviour is unsafe, disruptive, or harmful.
No refunds will be issued for removal due to behaviour.
7. Waiver of Liability
In consideration of my child’s participation, I release, waive, and discharge Lootie Patootie, its owners, employees, volunteers, contractors, and representatives from any and all liability, claims, demands, or actions arising out of or related to any loss, damage, injury, or illness that may occur during the Program, except in cases of gross negligence.
8. Photo/Video Consent
I give permission for my child to be photographed or recorded during the Program for marketing and social media purposes.
9. Refund Policy
Cancellations up to a week before the date chosen will be 50% refunded. Day-of cancellations for sickness will not be refunded.
10. Agreement & Signature
I have read and fully understand this waiver. I am the legal parent/guardian of the child named above, and I voluntarily agree to all terms.
