By submitting this application and/or remitting payment, I represent that I am authorized to bind my organization and agree to the CMF Sponsorship Agreement – Terms, Conditions & Rules (2026), including but not limited to non-refundability, no guarantee of attendance or ROI, Code of Conduct enforcement, public event/media acknowledgment, indemnification, limitation of liability, and binding arbitration provisions, all of which are incorporated by reference.

All sponsorship opportunities are subject to availability, internal review, and written approval by Concierge Medicine Today, LLC (“CMF”). Certain sponsorship categories may be subject to a prior renewal review period for returning sponsors. Submission of this application does not guarantee acceptance or category availability. This Agreement is not effective unless and until confirmed in writing by an authorized representative of Concierge Medicine Today, LLC.
CMF reserves the right to: Approve or decline any application at its discretion; Adjust sponsorship categories, benefits, or pricing prior to written confirmation; Correct typographical, clerical, or pricing errors prior to acceptance.
Our team will review and verify your selections within 5–10 business days. If approved, you will receive written confirmation and an electronic invoice (including applicable taxes and processing fees, if applicable). No sponsorship benefits, promotional placement, or event participation rights are activated until written confirmation is issued and payment is received.
