By submitting and signing below, I grant permission to Choose Chicago and the Coordinating Committee to use these materials in all advertising and promotion of Chicago Cabaret Week.

I hereby agree, if elected to participate in Cabaret Week 2025, to promote Chciago Cabaret Week using my social media networks and fan lists. I will participate in rehearsals and be punctual and respectful of my colleagues’ time. I shall respect the rules of the individual venue if my show is selected to participate.

Completion of this form does not guarantee that my show will be featured or selected in the Chicago Cabaret Week 2025. I understand that the Coordinating Committee and participating Venues will do their best to accommodate submissions by offering a diverse representation of the cabaret community of Chicago.
Submission requires a $35 payment unless you are a current member of Chicago Cabaret Professionals. We encourage you to join this important advocacy network for $35 per year!*
