Consent to Participate

I, the undersigned parent or legal guardian of the above-named child or children (“participant or participants”), hereby give permission for my child/children to participate in Favored Touch Events & Decor, LLC’s Kids Movie Night. I understand that the Event may include supervised activities and the viewing of age-appropriate films.

I acknowledge that participation in any group event involves a certain level of risk, including but not limited to minor injuries or allergic reactions. I confirm that my child/children is physically and emotionally able to participate in this Event.

Liability Waiver & Release

In consideration of my child’s/children's participation in this Event, I, on behalf of myself, my child/children, and our respective heirs, executors, and assigns, hereby fully release, discharge, and hold harmless Favored Touch Events & Decor, LLC, its owners, officers, employees, volunteers, and representatives from any and all claims, liabilities, damages, or demands arising from or related to my child’children's participation in this Event, including but not limited to personal injury, illness, property damage, or any other loss, whether caused by negligence or otherwise.

I understand that Favored Touch Events & Decor, LLC does not provide medical insurance and that any medical expenses incurred will be my sole responsibility.

Medical Treatment Authorization

In the event of an emergency and if I cannot be reached, I authorize Favored Touch Events & Decor, LLC representatives to seek emergency medical treatment for my child as deemed necessary by appropriate medical personnel. I agree to be responsible for any and all medical costs incurred as a result of such treatment.

Allergy & Food Acknowledgment

I understand that while all reasonable precautions will be taken to provide safe options, **Favored Touch Events & Decor, LLC cannot guarantee an allergen-free environment**. I accept full responsibility for notifying staff of any known allergies and assume all risk associated with food consumption during the Event.
I understand that snacks, beverages, and other food items may be provided during the Event.

Media Release

I grant permission for Favored Touch Events & Decor, LLC to photograph or record my child/children during the Event and to use such media for promotional purposes (social media, website, or marketing materials).I do not grant permission for my child’s/children's image or likeness to be used.
