AGREEMENT, ACKNOWLEDGMENT, AND RELEASEI, the undersigned, in consideration for my voluntary participation in Gam United Football Plus activities, do hereby acknowledge, understand, and agree as follows:1. Conduct & ResponsibilitiesI agree to maintain good standing as a participant and to conduct myself in a manner that reflects positively on Gam United Football Plus and its leagues. I understand that any violent, disrespectful, or unsporting behavior may result in dismissal from participation, with no refund of fees or dues. I accept full responsibility for any damage to property or persons caused by my willful or negligent actions.2. Assumption of RiskI acknowledge that soccer is a physical contact sport that involves inherent risks, including serious injury or death. I voluntarily assume all such risks arising from participation, whether during practices, games, training sessions, or travel to and from events. I certify that I am in good physical condition and have no illness or injury that would be aggravated by participation. Should my condition change, I will immediately inform my coach.3. Medical ResponsibilityI understand that Gam United Football Plus does not provide personal injury or medical insurance for participants. I am solely responsible for maintaining a current personal insurance policy to cover any medical costs arising from participation. I further agree that I am responsible for inspecting equipment and facilities and will notify a coach or supervisor of any unsafe condition prior to participation. Participation assumes consent to these terms.4. Photo and Media AuthorizationI grant permission for my photograph, image, likeness, and/or voice to be used by Gam United Football Plus in promotional materials, media coverage, broadcasts, or advertising, without compensation.5. Information Sharing AuthorizationI authorize Gam United Football Plus to provide an unaltered copy of this form to any league or tournament officials as needed to confirm eligibility for participation in affiliated soccer programs.6. Release and Waiver of LiabilityI hereby release, discharge, and hold harmless Gam United Football Plus, its directors, officers, administrators, employees, coaches, referees, volunteers, sponsors, partners, and any associated organizations or facilities from any and all claims, damages, liabilities, or expenses arising from my participation in any Gam United Football Plus activities. This release includes all successors, heirs, assigns, and representatives. ACKNOWLEDGMENT AND ACCEPTANCEI have carefully read this Waiver of Liability and Release Form, fully understand its contents, and voluntarily agree to its terms. I understand that by signing this document, I am giving up substantial legal rights on behalf of myself, my heirs, and my assigns.
