LATE FEE- $15 LATE FEE PER APPLICANT WILL BE ASSESSED AFTER FEBRUARY 23RD, 2026, TO COVER THE COST OF RUSH UNIFORMS. SAAA RETAINS THE RIGHT TO REFUSE APPLICANTS AFTER THE DEADLINE IF ROSTERS ARE FULL. (Any late orders may not receive their uniforms prior to picture day.)

Responsibility Clause as the parent or legal guardian, I agree to be present or have an appointed family member or friend in attendance for all games/practices/events. If I am unable to stay for the duration of the event or must leave for a brief period of time, I agree to inform my child’s head coach and to provide him a contact number in case of an emergency. I will be responsible for the return of all uniforms and equipment regardless of condition. I understand that my child must participate in all SAAA fundraisers. If the money is not paid for these fundraisers, I understand that legal and disciplinary measures may be taken. I the parent/legal guardian, also agree that my child's uniform will not be distributed unless a concession check or cash is submitted to the organization. Once the season is complete, cash payments will be returned and checks will be shredded. *

Liability WaiverI the parent/legal guardian of the above named child, hereby give my approval for my child to participate in activities sponsored by SAAA during the current season. I the parent/legal guardian assume all risks and hazards incidental to the conduct of the activities and transportation to and from the activities. I do hereby release and hold harmless SAAA and it affiliates, the organizers, sponsors, officers, and supervisors appointed by them. I likewise release from responsibility any person transporting my child to or from these activities. I will furnish a certified birth certificate upon request. SAAA does provide secondary medical and liability insurance on all players and adult helpers but the individual’s primary insurance must be used first. All photos, info, stats can be used by SAAA for newspaper printing and web/social media purposes. *
