Please alert us of a uniform order for a newly registered player after June 1.
ATTENTION PLEASE: If you registered during the regular registration period of May 1 thru June 1, then your order will be placed by your team manager, and you should NOT use this form.

For any other time of the year, once you have registered, you may use this form to request a uniform.

FULFILLMENT TIME FRAMES:

Peak Times (May 1 thru Sept 10) - fulfilled by club timelines
Non Peak Times (Sept 10 thru April 30) - allow 1 week

SHIPPING OPTION:
Uniforms will be available for pickup during regular Club Shop hours at the West Bank Ken Lovelee Clubhouse.

You can also choose shipping ($20).

Shipping to an Oklahoma address adds 2 business days to the fulfillment timeline.
Shipping outside of Oklahoma will add 3-5 business days to fulfillment.
