AAU Membership

Players must have an AAU Membership to participate. Register for an AAU ID number at https://aausports.org/membership.

Emergency Contact & Health Insurance Information

Parental Permission For Emergency Treatment
and Release of Liability

I, Parent/Guardian, give consent for the Maui Versatility Basketball League to secure any and all necessary emergency medical care for my child.

In the event of illness or accident, I give my permission for emergency treatment by qualified medical personnel for my child, and I authorize the person in charge to take my child to:

I, Parent/Guardian, acknowledge that I have read and understand the following:

Although the safety of all sport activities is the primary concern, indoor sport activities with Maui Versatility Basketball League may cause injuries and/or death. I expressly assume the risk of injury, death, and/or illness arising from any cause, and agree to waive the right to pursue any claim against the Maui Versatility Basketball League and the persons in charge.

COVID-19 Acknowledgment:

I, Parent/Guardian, acknowledge that I have read and understand the following:

I understand the contagious nature of COVID-19 and the risks associated with participating in basketball activities during the ongoing pandemic.
I am aware of and understand the preventive measures recommended by health authorities, such as maintaining physical distance, wearing masks, practicing proper hygiene, and following any additional guidelines provided by the basketball facility or organization.
I understand that despite these precautions, there is still a risk of contracting COVID-19 while participating in basketball activities.
Assumption of Risk:

I voluntarily assume all risks related to exposure to COVID-19, including but not limited to infection, illness, or injury, that may result from participating in basketball activities organized or facilitated by the Maui Versatility Basketball League.

Release of Liability:

In consideration of being allowed to participate in basketball activities, I, Player and Parent/Guardian, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and covenant not to sue Maui Versatility Basketball League, its officers, directors, employees, agents, volunteers, or representatives from any and all claims, liabilities, damages, or demands arising from or related to COVID-19, including those caused by the negligence of the released parties, whether known or unknown, foreseen or unforeseen, arising out of or in connection with my participation in basketball activities.

Indemnification:

I agree to indemnify, defend, and hold harmless Maui Versatility  Basketball League, its officers, directors, employees, agents, volunteers, or representatives from any and all claims, liabilities, damages, or demands arising from or related to COVID-19, including those caused by my negligence or misconduct.

Medical Authorization:

I authorize Maui Versatility League to seek medical treatment on my behalf in the event of an emergency if I am unable to communicate my consent.

Acknowledgment of Understanding:

I acknowledge that I have read this waiver form, understand its contents, and voluntarily agree to its terms.

MEDIA CONSENT, RELEASE, AND WAIVER

I hereby give consent to Maui Versatility League to photograph, videotape, or otherwise digitally record and use images and/or sound recordings of myself or my child or children (if applicable) to use in any public media, including radio, television, internet, social media, print or in any of the organization’s or its partners’ publications, productions, or posts. I understand that the intended use of such images and information is solely for the purpose of advertising, marketing, fundraising and/or the promotional and public awareness purposes for the organization. I hereby waive any rights or interest in the images or recordings, as contemplated in this release.

I acknowledge that this consent to use images and/or recordings is being made solely for the benefit of the organization and comes without any expectation of monetary compensation or other benefit to me. To the extent that any benefit accrues or might accrue to the organization from the use of images or information, I hereby and forever waive any interest in or claim to such benefits.

I hereby release and forever discharge the organization (including without limitation all corporate affiliates and officers, directors, trustees, donors, employees, agents and volunteers) from any and all claims, liability, actions, suits, demands, costs, expenses or indebtedness arising out of, related to, or in any way connected with the use of images and materials described herein, and I hereby waive all rights and interest in and to such information and materials.

I further acknowledge that there is no guarantee that any or all of the participants’ images or recordings will be used in any released media.

AAU MEMBERSHIP AND SEASON GUIDELINES

 

Players/Guardians accept and understand that in order to participate as a coach and/or player, they are required to have a valid AAU Membership per our insurance requirements. 

Coaches need to submit a roster of their team a week prior to start of season. Coach AAU numbers and all player AAU numbers must be listed on the roster a week prior to the start of the season. Final roster with changes and jersey numbers are due 2 days prior to the start of the season. 

AAU Numbers will be checked a week before the start of the season. Any coaches or players not in compliance will be noted. They will have until 5pm of the Thursday before the start of the league season to send correct/updated AAU Membership numbers.
