Otsego Bands is excited to offer the opportunity to purchase customized jackets this year! These jackets are only offered every 2 years.

Jacket prices are $85 (S-XL) and $87 (2XL-3XL). Payments must be submitted with this order form by Friday, December 5th. Please use one order form per person. Only one instrument is allowed.

If a relative wishes to purchase a jacket, only the relative's name will appear on the jacket unless the instrument is written below.
