I understand that participation in Passaic Sports League involves physical activities, sports, and outdoor play that carry a risk of injury. I acknowledge and accept that my child’s participation is voluntary and that all activities involve some level of risk, including possible injury or property damage.

I agree that Passaic Sports League, its staff, coaches, volunteers, and organizers are not responsible for any injury, accident, loss, or damage that may occur during activities, on or off the property, except in cases of gross negligence or willful misconduct.

I understand it is my responsibility to ensure my child is physically able to participate and that they have any necessary medical clearance. I also give permission for staff to seek emergency medical care for my child if needed and agree to be responsible for any related costs.
