    This form is ONLY for transferring a currently registered athlete from one Arizona club to another Arizona club.

    The $15 transfer fee is NON-REFUNDABLE. Refunds will not be issued for incorrect submissions, ineligible athletes, duplicate requests, or any other reason.

    Athletes transferring from a club in another state to a club in Arizona should use the transfer form available in the Document Library on our website.
