The Summit Park District is committed to conducting its recreation programs and activities in a safe manner and holds the safety of participants in high regard. We continually strive to reduce risks and insist that all participants follow safety rules and instructions designed to protect everyone’s wellbeing.

However, participants and parents/guardians of minors must recognize that there is an inherent risk of injury when choosing to participate in recreational activities and programs.

You are solely responsible for determining if you or your minor child/ward are physically fit and appropriately skilled to participate in the program. It is strongly recommended that you consult a physician prior to participation, especially if the participant has experienced illness, injury, disability, or impairment.

Warning of Risk:
Recreational programs and activities are intended to challenge and engage participants physically, mentally, and emotionally. Despite careful preparation, instruction, medical advice, conditioning, and the use of proper equipment, there is still a risk of serious injury.

Certain risks, dangers, and injuries may occur, including but not limited to:

Slipping, tripping, or falling
Collisions or contact with people or objects
Weather-related hazards
Improper skill level or conditioning
Carelessness or horseplay
Premises or equipment defects
Inadequate supervision, instruction, or officiating
It is impossible for the Summit Park District to fully eliminate all risks. Participants must acknowledge and accept these inherent risks.

Waiver, Release, and Assumption of Risk:

By registering for and participating in this program/activity, you understand that you are assuming the risks associated with participation, including all risks relating to transportation services when provided.

I acknowledge that I and/or my minor child/ward may sustain injuries or damages arising out of participation in this program/activity. I voluntarily assume full responsibility for any and all injuries, damages, or losses, regardless of severity, that may be sustained.

I further agree to waive, release, and relinquish all claims against the Summit Park District, its officers, agents, volunteers, and employees, arising from participation in this program/activity.

I do hereby fully release and forever discharge the Summit Park District from any and all claims for injuries, damages, or losses that may accrue to me or my minor child/ward in connection with participation in this program/activity.

By signing below, I acknowledge that I have read, understand, and agree to the above waiver, release, and assumption of risk.

By acknowledging and signing below, I certify that I am providing an electronic signature that has the same legal effect as an original handwritten signature. This electronic signature is equally binding as an original manual paper signature.
