Certification Acknowledgment for Individual CPGE™ Registration

I have reviewed the requirements outlined in the Handbook for the certification I am applying for (available at certplace.com) and acknowledge my responsibility to understand its contents. I certify that the information provided in this application is true, accurate, and complete. I authorize Certplace to conduct inquiries or investigations as deemed necessary to verify my credentials and professional standing for eligibility to take the certification exam. I understand that providing false information may result in the denial or revocation of the credential.  

I acknowledge that Certplace reserves the right to disqualify me from taking, continuing, or receiving examination results if it determines, at its sole discretion, that I engaged in inappropriate conduct during the examination process

I understand that all fees paid are non-refundable.  

After earning the credential, I accept responsibility for fulfilling all requirements to maintain it, including completing the necessary educational contact hours within the specified timeframe and applying for certification renewal. I also understand it is my duty to inform Certplace of any updates to my contact information. I acknowledge that Certplace will treat candidate information, certification status, application contents, and related documentation as confidential, except as necessary for program administration. Candidate information, status, and exam results will be disclosed only to me directly by Certplace staff, not to any third parties. Upon my request, written or verbal, Certplace may release my certification status. Finally, I understand that Certplace does not disclose examination scores or the number of exam attempts.

Acknowledgment for Registrant Completing the CPGE™ Form on Behalf of a Candidate

I certify that the information provided in this application is true, accurate, and complete to the best of my knowledge. I authorize Certplace to conduct inquiries or investigations as deemed necessary to verify the credentials and professional standing of the candidate(s) for eligibility to take the certification exam. I understand that providing false or inaccurate information may result in the denial or revocation of the candidate’s credential.

I acknowledge that Certplace reserves the right to disqualify the candidate(s) from taking, continuing, or receiving examination results if it determines, at its sole discretion, that inappropriate conduct occurred during the examination process.

I understand that all fees paid are non-refundable.

If the candidate(s) earns the credential, I accept that it is their responsibility to fulfill all ongoing requirements to maintain it, including completing the necessary educational contact hours within the specified timeframe and applying for certification renewal. I also commit to informing the candidate(s) of their duty to notify Certplace of any updates to their contact information.

I acknowledge that Certplace will treat all candidate information, certification status, application contents, and related documentation as confidential, except as necessary for program administration. Candidate information, status, and exam results will be disclosed only to the candidate(s) directly by Certplace staff, not to any third parties. Upon written or verbal request by the candidate(s), Certplace may release their certification status. I understand that Certplace does not disclose examination scores or the number of exam attempts for any candidate(s).
