Acknowledgment of Risk
I acknowledge that participation in martial arts training at Urban Edge Martial Arts involves inherent risks, including but not limited to physical injury, disability, or even death. I understand that these risks may arise from the nature of the activities, the actions of other participants, or the conditions of the training environment.
Assumption of Risk and Release
I voluntarily assume all risks associated with my participation in martial arts classes, training, sparring, and related activities at Urban Edge Martial Arts. I hereby release, waive, discharge, and covenant not to sue Urban Edge Martial Arts, its owners, instructors, employees, and agents from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me while participating in these activities.
Medical Fitness
I certify that I am physically fit and capable of participating in martial arts training. I have disclosed any medical conditions or injuries above that may affect my participation.
Compliance
I agree to follow all rules, instructions, and safety guidelines provided by Urban Edge Martial Arts instructors and staff.
No Refund Policy
I understand and agree that all fees paid to Urban Edge Martial Arts, including but not limited to membership fees, tuition, and other charges, are non-refundable.
Cancellation Policy
Recurring Memberships: I may cancel my recurring monthly membership by providing a minimum of thirty (30) days written notice to Urban Edge Martial Arts prior to my next billing date. I am responsible for all payments within this 30-day notice period, and no prorated refunds will be issued for mid-cycle cancellations.
Fixed-Term Contracts: I understand that my membership is for an initial fixed term. Early cancellation of this contract outside of the specific conditions outlined in this document (e.g., relocation) is not permitted unless agreed upon in writing by management and may be subject to an early termination fee.
Membership Termination Due to Relocation
I agree to notify Urban Edge Martial Arts promptly if my primary residence moves to a location greater than 25 kilometers from the school. Upon providing proof of the new address (e.g., a driver’s license or utility bill), my membership may be terminated without penalty, subject to the terms of my specific membership agreement. Early termination fees for fixed-term contracts will be waived under this condition.
Consent for Emergency Treatment
In the event of an emergency, I authorize Urban Edge Martial Arts to obtain medical treatment on my behalf if I am unable to do so.
Video and Photo Release
I hereby grant Urban Edge Martial Arts, its representatives, instructors, employees, and agents the irrevocable right and permission to take photographs, video recordings, and other media of me during training, classes, events, or related activities. I consent to the use of my image, likeness, voice, and appearance in any and all promotional, marketing, instructional, or advertising materials, whether in print, digital, or online format, now or in the future, without compensation.
I understand that Urban Edge Martial Arts owns all rights to such media and may edit, reproduce, display, or distribute it for lawful purposes. I waive any right to inspect or approve the finished product or material and release Urban Edge Martial Arts from any claims related to the use of my image or likeness.
