UNI FOR HAIR: Waiver and Consent Form  &#13;&#10;Consent for Photography and Intellectual Property Rights&#13;&#10;I, the undersigned, hereby grant permission to UNI FOR HAIR LLC to take photographs and/or video recordings of my participation in any MEETINGS, LOC CLASSES and any related LIVE VIRTUAL activities. I understand that these images may be used for promotional purposes, including but not limited to social media, website content, and marketing materials.&#13;&#10;&#13;&#10;2. Intellectual Property Rights:&#13;&#10;- I acknowledge that all photographs, videos, and educational materials produced by &#13;&#10;UNI FOR HAIR LLC during all classes are the intellectual property of UNI FOR HAIR LLC.  &#13;&#10;- I agree not to reproduce, distribute, or use any of the materials or images without explicit written permission from UNI FOR HAIR LLC.&#13;&#10;&#13;&#10;3. Payment Policy:&#13;&#10;- I understand that all payments made for classes, including the $333 LOCS 101 Scholarship Class fee are nonrefundable.  &#13;&#10;- I understand I must purchase ALL required props and tools needed for the class on UNI FOR HAIR Amazon Affiliate Link. &#13;&#10;OR&#13;&#10;- I understand I must purchase ALL required props and tools needed for the class. &#13;&#10;- I acknowledge the needed props and tools list is in the UNI FOR HAIR LLC Amazon Affiliate Link.&#13;&#10;- I acknowledge that participation in the class includes FREE membership in LOCS 101 ALUMNI COMMUNITY, hosted in &#13;&#10;UNI FOR HAIR HUB. This membership enables alumni to attend weekly scheduled LIVE, “FIX-IT FRIDAY”, where hands on training will lead the collected weeks from the LOCS 101 Alumni Community through uploaded texts, pictures and videos. &#13;&#10;&#13;&#10;**4. Release of Liability:**  &#13;&#10;- I release UNI FOR HAIR LLC and its representatives from any claims, demands, or liabilities arising out of or in connection with the use of my images or participation in the class.&#13;&#10;&#13;&#10;**5. Classroom Entry Requirements:**  &#13;&#10;- All students must present a valid ID and have a phone to sign in and enter the classroom.&#13;&#10;&#13;&#10;**6. Transportation Policy:**  &#13;&#10;- There is no transportation provided to or from the school. Public transportation is available in the form of a bus. Students are responsible for making arrangements to be picked up within 30 minutes of the end of the class time.&#13;&#10;&#13;&#10;**7. Video Requirement:**  &#13;&#10;- As a prerequisite for the scholarship, all participants must provide a 30 to 60-second video to UNI FOR HAIR about their experience.
