ABVO Maintenance of Certification Application Form

Please submit this form for both mandatory or optional applications. Those who are successful with their MOC renewal will receive a confirmation letter for their office and be allowed to download a special MOC graphic for promotional use.
Mandatory applications for the MOC of Diplomates boarded in 2020 must be received through the ABVO/MOC online application form by December 31st, 2025 at 11:59pm MST. Mandatory applications will be reviewed after this deadline.

Voluntary applications for the MOC of Diplomates boarded prior to 2015 must be received through the ABVO/MOC online application form by March 1st, 2026 at 11:59pm MST. All Voluntary applications will be reviewed after the March 1st deadline.

For both mandated and voluntary applicants, the designated application windows are the only times submissions can occur—November to December for mandatory candidates and November to March for voluntary candidates.

Mandatory candidates may only submit at their five-year mark. Early submission does not delay or reset the next five-year deadline; rather, it restarts the cycle, which could result in having to pay for submission more frequently.

Voluntary applicants, on the other hand, are not tied to a specific year cycle but must still submit within the designated application window.

Note for completing spreadsheet: First time mandated applicants should add credits for MOC from the months of the calendar year subsequent to achieving Diplomate status to the first column on the MOC credit spreadsheet with the first full calendar year data.
