Liability Waiver
Names of Programs to be Released: Vancouver Ravens Baseball Association
Activities: Baseball training/practices, scrimmage games, exhibition games, Tournaments, and/or related baseball activities, Travel
Location of Activity/Program: Various private and public baseball fields and indoor practice facilities located in Washington and Oregon and/or any state the team travels to play a game/tournament
Assumption of Risk: I understand that participation in Vancouver Ravens Baseball activities involves inherent risks, including but not limited to physical injury, illness, property damage, and the risk of contracting contagious diseases. I voluntarily and knowingly assume all such risks associated with my participation.
Release and Waiver: I, for myself and my heirs, hereby release, discharge, and hold harmless Vancouver Ravens Baseball, its officers, directors, coaches, volunteers, and all associated personnel, from any and all claims, demands, actions, or causes of action, whether for personal injury, property damage, or otherwise, arising out of or in connection with my participation in any and all activities listed above, including any actions or negligence on the part of Vancouver Ravens Baseball and its personnel.
Medical Treatment Authorization: I authorize Vancouver Ravens Baseball and its representatives to seek and obtain any necessary medical treatment or attention in the event of an injury, illness, or medical emergency during any activities my player attends. I understand that Vancouver Ravens Baseball will make reasonable efforts to contact me or my emergency contact before seeking medical treatment, but they may proceed with necessary medical care if immediate attention is required.
Player affidavit: I hereby give Vancouver Ravens Baseball permission to use my player's name, picture and likeness (without right of approval) for any use to promote the Vancouver Ravens organization, practices, games, tournaments and player events in any and all publications that they may desire including but not limited to the Vancouver Ravens website, Facebook, Instagram, TikTok, YouTube, X and Threads.
I acknowledge that I have read and fully understand this agreement, which involves surrendering and releasing valuable legal rights. I enter into this agreement freely and voluntarily. Furthermore, the undersigned understands that their participation or attendance in the activities mentioned above, or their child’s participation or attendance in the outlined program, is completely voluntary.
