REGISTRATION FORM

This registration form can be completed by a single athlete or a team representative. 

Payment is required at the end of this form before athlete(s) will be registered.

IF YOU ARE REGISTERING A LARGE TEAM (10+ athletes) - WE RECOMMEND YOU ORGANIZE YOUR ATHLETES BY DIVISION (ie. Cadet - Freestyle, Junior - Freestyle etc) before proceeding as you will be asked to enter athletes one division at a time.  

• Scratches (Withdrawals)- Registrations can be canceled and will be fully refunded if submitted BEFORE 6pm FRIDAY, DECEMEBER 12th.  A Matmen representative will issue an online refund accordingly.
  
  
• Payment - Payment MUST BE COMPLETE before athletes will be entered into the tournament.
