Requirements for CMPA Student Athletes
1. Pay sports participation fee prior to the first game of the season.
2. Register for YOUTH ON COURSE (required).
3. Prior to the first practice, 9th and 11th grade students must have a Health History and Physical Exam completed. Use the form linked in the confirmation email.
4. Demonstrate academic responsibility by having no missing assignments throughout the sport season and maintain passing grades of 2/C or higher in all classes.
5. Student athletes are required to participate in athletic fundraisers during their season in support of the CMPA athletic program.
6. Parental support is needed during each season. Volunteer opportunities will be made available through sign-up in the FACTS Family Portal.
After reviewing the above stated requirements,
CMPA Student Athlete Code of Conduct
1.) I agree to use God honoring language and communication. Profanity, crude language, or vulgar speech is not permitted. The team coach will address any violations with the athlete and their parents. A pattern of violating this expectation may result in suspension or removal from the team. Speech should reflect Christ and build others up (Ephesians 4:29).

2.) I agree to have Christ-centered conduct. CMPA student-athletes are called to represent Jesus Christ both on and off the field (II Corinthians 5:20). This means exhibiting respect, humility, integrity, and kindness toward teammates, coaches, opponents, officials, and all others at all times.

3.) I agree to respect authority. Athletes must show honor and obedience to coaches, teachers, referees, and school staff. - Disrespect or defiance will be addressed and may result in reduced playing time or other disciplinary action. - Regular attendance at practices is expected. Unexcused absences may affect eligibility to play in games.

4.) I agree to maintain academic eligibility and Integrity. CMPA emphasizes academic excellence alongside athletic achievement. Student-athletes must: - Memorize the assigned weekly portion of the memory verse. - Have no missing assignments. - Maintain a minimum grade of "2/C" in all core subjects.

5.) I agree to accept correction and be held accountable. A student referred to the office for disciplinary action my be suspended for half of the next game. - Repeated offenses or lack of repentance may lead to further consequences, including potential removal from the team. - CMPA believes discipline is part of discipleship and spiritual growth (Hebrews 12:7-11)

6.) I understand that participating in CMPA Athletics is a privilege and a responsibility. I agree to uphold the expectations set forth in this Code of Conduct and strive to honor Christ in all areas of my life as a student-athlete.

Notice of Risk for Student Athletes
We realize that such activity involves the potential for injury that is inherent in all sports. We acknowledge that even with the best coaching, use of the most advanced protective equipment, and strict observance of rules, injuries are still a possibility. On rare occasions these injuries can be severe as to result in total disability, paralysis or even fatal. We, the typed undersigned, understand the dangers of practicing, playing and participating in sports. We specifically acknowledge that we have carefully read and understand this Notice of Risk for Student Athletes and will not hold Calvary Meridian Prep Academy responsible for any unforeseen injury.
I have read and agree to the Notice of Risk and Waiver of Liability.
Commitment for Athletes and Spectators
All competitions must be conducted with a high standard of courtesy, fair play and sportsmanship. All of those involved share responsibility. Athletes must demonstrate qualities of courtesy, good sportsmanship, and Christ-like attitude. Spectators, both students and adults, need to demonstrate courtesy, good sportsmanship, and Christ-like attitude by cheering in a positive and appropriate manner for their team and never against the opponents. This is evidenced by the absence of booing or loud negative outbursts to either team or referee. Spectators must show proper acceptance of official's judgement and the coach's decisions. Anyone not observing this Code of Conduct will be asked to leave and must contact the Athletic Director and Principal before returning to any other school events.

We give our consent for coaches or school personnel to use their best judgement in securing medical aid and ambulance service in case the parent/guardian can not be reached.
Athlete Transportation
Parents are responsible for the transportation to and from games; parents may choose to arrange carpools on their own accord, but athletes are not allowed to transport other student athletes to away games. The coaches must have written notice of who is allowed to transport the student other than the student's own parent.
