Liability Waiver and Release of Claims

Assumption of Risk and Waiver of Liability
I hereby release, waive, discharge, and covenant not to sue the CHS Football Booster Club, Crismon High School, Queen Creek Unified School District, their officers, employees, volunteers, agents, representatives, sponsors, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or my property while attending or participating in this event.
Indemnification
I agree to indemnify and hold harmless the released parties from any loss, liability, damage, or cost they may incur due to my attendance or participation in the event.
Minors
If the participant is under the age of 18, this waiver must be signed by a parent or legal guardian, who agrees to all terms of this waiver on behalf of the minor.
Acknowledgment and Signature
I have read this waiver and release carefully, fully understand its terms, and sign it freely and voluntarily. I understand that by signing this document, I am giving up substantial legal rights.
