Release & Liability
In Consideration of my/our child being allowed to participate in any way for the Brawlers, it's related events and activities, the undersigned acknowledges, appreciates, and agrees that:
In acknowledgment of my child’s participation in recreational volleyball at Zatzman gym, I understand and agree to the following:
I acknowledge that Zatzman gyms and the game of volleyball may have features, amenities, circumstances and conditions that may carry inherent risk, including risk of bodily injury, illness or disease, disability, or death. I understand that the description of the risks in this Liability Waiver is not complete and that other risks or events that are known or unknown, anticipated or unanticipated. You agree that by participating in the playing of recreational volleyball you voluntarily and willfully assume those risks and assume full responsibility for the choices you make before, during and after the games and use of Zatzman gyms. In the event of any damage or loss of property in association with your child’s participation in the volleyball games you acknowledge and agree to be held liable for any and all costs arising from those acts. You agree to indemnify and hold harmless Zatzman gyms their respective affiliates or agents, Katie Goodz, and the children and parents participating in the recreational volleyball games for and from all claims for property damage, personal injury, illness or disease, or all losses, claims, damages, liabilities, and related expenses, including the reasonable fees, charges and disbursements of any counsel for any indemnitee, incurred by or against any indemnitee arising out of, in connection with, or as a result of participation in the recreational volleyball games. Having read this Liability Waiver, the undersigned agrees to these terms, and further agrees that no oral representations, statements or inducements apart from the foregoing written agreement have been made.
I have read this release of liability and assumption of risk agreement. I fully understand its terms. I understand that I have given up substantial rights by signing it, and sign it freely and voluntarily without any inducement.
If a child leaves the event for any medical reason, they cannot return without medical clearance from a physician. At any time, a coach can require medical clearance of a player to be able to participate or play at their own discretion.
