Parent/Guardian authorization: This health history is accurate to the best of my knowledge and the person herein described has my permission to engage in all planned Camp Cue activities, except as noted by the examining physician and me. In the event that neither I nor an emergency contact described above can be reached in an EMERGENCY, I hereby give permission to the health care provider selected by the Camp Cue nurse and directors to hospitalize, provide proper treatment for and order injections as necessary.*

Medical Transportation and Supervision Policy: In the event that a camper requires transportation to a hospital or emergency medical facility, the primary guardian agrees to begin making immediate arrangements to have themselves or an authorized representative (such as an emergency contact, co-guardian, or family member) meet the camper at the hospital as soon as possible. Camp Cue staff will accompany the camper during transportation to the hospital if necessary but are not able to remain with the camper overnight or provide extended supervision at the medical facility. If no guardian or authorized representative is able to arrive in a timely manner, and extended supervision becomes necessary, the guardian understands that additional costs may be incurred and agrees to assume full responsibility for those costs.*
