    Player Self-Assessment Form | Bi-Monthly Performance Review
  • The Player Self-Assessment Form is a core part of CVC’s high-performance pathway.

    Completed every two months in alignment with our fitness testing schedule, it is designed to help you reflect honestly on your development, identify areas for growth, and take ownership of your performance both on and off the court.

    By completing this form, you will:

    • ✅ Build greater self-awareness around your technical skills, physical readiness, mental approach, and contribution to team culture.
    • ✅ Provide coaches with valuable insights to shape your development and tailor feedback to your specific needs.
    • ✅ Create a documented record of your growth that informs long-term progression within the club.

    This assessment is mandatory for all registered players — including shadow athletes — and must be submitted within 7 days of each testing block. Failure to complete the assessment may affect your eligibility for selection in upcoming competitions or advancement to higher squads.

    Approach this form with honesty and intention. It is not a test — it’s an opportunity to measure where you are, set a clear focus for the next stage of your development, and play an active role in your journey as a high-performance athlete.

    This section:

    • Help players reflect on their on-court performance and game understanding.
    • Identify strengths and weaknesses within key skill categories.
    • Provide coaches with data they can cross-reference with objective match/video analysis.
    • Create a consistent scoring language that lets you track development over time.

    Rating Scale Definition:

    Rating Description
    1 – Needs Major Improvement Regular errors, unreliable under pressure
    2 – Developing Basic execution but unreliable or incomplete
    3 – Competent Solid, reliable, but not yet impactful
    4 – Strong Consistently effective, occasional high-impact plays

  • Section 3: Bi-Monthly Fitness Results

  • PURPOSE OF THIS SECTION

    This section records your bi-monthly fitness testing results. It provides an objective snapshot of your physical readiness against CVC’s performance standards and helps track your progression over time. Accurate data here allows coaches to correlate your physical condition with on-court performance and tailor training plans accordingly.

  • Performance Summary Score – Interpretation

    This score represents your overall physical readiness based on the results of all fitness tests, weighted for their importance. It provides a single, easy-to-track number that reflects how well you meet CVC’s elite performance benchmarks:

    • 🔴 Below 100 – You have not yet met the minimum performance benchmark for your tier. Improvement is required before you can consistently compete at the required level. Use your individual component scores to identify which areas are holding you back.
    • ⚪ Exactly 100 – You are at the baseline performance level required for your current squad. This is the expected standard to compete and remain in contention, but to progress to higher squads or representative opportunities, continued improvement is needed.
    • 🟢 101 - 110 – You are performing above standard, demonstrating a physical profile that will positively influence match outcomes. You are trending toward elite readiness.
    • 🔵 111 - 120 – You are significantly exceeding the standard and performing at a level consistent with top athletes in your competition tier. This score indicates a strong case for progression, selection, and leadership opportunities.
    • 🥇 More than 120 – You are operating at or above the physical profile of an elite athlete. This score demonstrates exceptional readiness, strong training habits, and the physical base required for national or professional development pathways.
