Informed Consent — Novaskyn Treatments I understand that the treatments offered at Novaskyn — including manual lymphatic drainage, red light heat therapy, ultrasonic cavitation (lipocavitation), and body sculpting massage — are non-medical wellness treatments performed by massage therapists under registered therapists supervision. I acknowledge that: (a) these treatments are not a substitute for medical care; (b) results vary by individual and depend on consistency, hydration, and lifestyle factors between sessions; (c) Novaskyn does not diagnose, treat, or cure medical conditions.I confirm that the health information I have provided is accurate and complete. I agree to inform Novaskyn of any changes to my health status that may affect my treatment.I understand that in the event of discomfort, pain, or adverse reaction during a session, I will inform my therapist immediately and the session will be paused or stopped as appropriate.I release Novaskyn, its owners, employees, and associates from any liability for adverse effects resulting from the accurate disclosure of my health information and compliance with contraindication guidelines.

As part of the registration process, we will collect your credit card information. This information is necessary for the following purposes: 1. Account Creation: Your credit card details will be used to set up your membership account securely and ensure a smooth registration process. 2. Membership Purchase: We will process your payment for the selected membership plan using your credit card information. This will allow you to access to your account so you can access our booking system to create your appointments. We take your privacy seriously. All credit card information will be stored securely on your account and used only for the purposes outlined above. We will never share this information with third parties. Your credit card information will stay on your account as it is needed as per our Booking Policy and Cancellation Policy. All appointments must be reserved with a credit card. By providing your credit card information, you consent to its use for the purposes mentioned above. If you have any questions or concerns, please do not hesitate to contact us.

Billing: My selected membership fee will be charged monthly on the same calendar date as my enrollment date, via Square invoice sent to my email address. I authorize Novaskyn to charge my payment method on this recurring basis. Sessions: Monthly sessions included in my membership do not roll over to the following month. Unused sessions expire at the end of each billing period. Pause policy: I may pause my membership once per year for a maximum of 1 calendar month, provided I notify Novaskyn at least 5 days before my next billing date. During a pause, no sessions are available and no charge is processed. Cancellation: This is a contract of 3 months, after that I may cancel my membership at any time by notifying Novaskyn at least 15 days before my next billing date. Cancellations received less than 15 days before billing will result in one final charge; the membership will then close at the end of that paid period. Price changes: Novaskyn may adjust membership pricing with 30 days written notice. Active members are not subject to price increases during the first 12 months of enrollment. No refunds: Monthly membership fees are non-refundable once charged. Unused sessions within a paid period are forfeited upon cancellation. Location: Membership sessions may be used at the Novaskyn location . Availability subject to scheduling.

As part of any Novaskyn Program and standard nurse or therapist intake protocol, Novaskyn takes standardized photographs (front, back, and sides) and body measurements at intake, mid-program, and at program completion for clinical documentation purposes.These photographs and measurements are stored securely in your private clinical dossier and are not shared without your explicit consent.
