Use this form if you are unable to attend a scheduled competition game.
CVC requires all players to provide a minimum of 72 hours’ notice if they are unavailable for a scheduled match. Adequate notice allows coaching staff to adjust line-ups, notify shadow players, and plan strategically without compromising team performance or competitive standards.
Where players are unable to meet this notice requirement, an Administrative Absence Charge will apply. It reflects the additional administrative workload, logistical disruption, and competitive impact caused by late withdrawals. Its purpose is to encourage responsible communication, strengthen team reliability, and uphold the integrity of selection and game-day planning.
The following tiered structure applies:
| Notice Provided |
Administrative Absence Charge |
| More than 72 hours |
❌ No charge |
| Between 72 and 48 hours |
💳 $4.97 |
| Between 48 and 24 hours |
💳 $14.97 |
| Less than 24 hours |
💳 $24.97 |
| No show (no form submitted) |
💳 $49.97 |
Important: This Administrative Absence Charge is in addition to any fines or sanctions that may be imposed under the CVC Regulatory Fines & Compliance Framework.
Failure to submit this form will be treated as a no-show and charged accordingly.
CVC recognises that genuine emergencies (e.g., sudden illness, injury, or significant personal circumstances) can occur. These will be reviewed on a case-by-case basis. All submissions require payment of the applicable charge, which will be reimbursed if your reason is approved.
Repeated late withdrawals or absences will trigger a performance and commitment review. This may involve a formal check-in with coaching staff to address underlying issues and help get you back on track.
