Thank you in advance for registering your youth for the 76th Supreme Convention's Supreme Youth Department of International Free and Accepted Modern Masons, Inc. and Order of the Eastern Star, to be held July 11–18, 2026, in Baton Rouge, Louisiana.

The 2026 Supreme Convention Youth Registration is $85 per youth which covers participation in all youth-department activities and sessions during the 76th Supreme Convention's Supreme Youth Department. This fee also includes access to youth programs, materials, and scheduled events. Each youth must be registered individually under a Youth Worker or assigned Chaperone. The total amount charged will include the applicable transaction fee.

This form is for the registration of youth and by a Parent, Guardian, or Authorized Youth Worker. Please take a few minutes to complete all required sections carefully. Accurate information helps us ensure the comfort, safety, and full participation of each youth during the Convention.

Important Notes:
- Youth registration is $85 per youth, plus any applicable transaction fees.
- Registration includes access to all official youth sessions, activities, and materials.
- Please provide accurate medical and emergency contact information to help us support your youth’s well-being during the event.

Section IV - Permission & Consents
I give permission for my child to attend and participate in all youth-related activities during the International Free & Accepted Modern Masons, Inc. and Order of the Eastern Star's 2026 Supreme Convention in Baton Rouge, Louisiana.

I understand that my child will be under the supervision of designated Youth Workers and authorized IFAMM/OES leaders. I release and hold harmless International Free and Accepted Modern Masons, Inc. and Order of the Eastern Star, its officers, volunteers, and affiliates from any liability resulting from participation, except in cases of gross negligence.

I also grant permission for my child’s photo or video to be used in Official IFAMM/OES publications, social media, and promotional materials unless I have provided written notice requesting otherwise.
