CPD External Accreditation Application Form (updated Oct2025) Logo
  • CAEP Royal College Accreditation Application Form

  • Important Directives

To ensure timely review and approval, this CPD Accreditation/Certification Application form—along with all required supporting documents—must be submitted at least 8 weeks prior to the start date of the activity. Please note the following:

    • Accreditation/certification approval is not retroactive.
    • No references to the Royal College of Physicians and Surgeons of Canada (RCPSC), MOC Section 1, or MOC Section 3 credit approval may be made until official approval is granted. Phrases such as “credits pending” or “applied for” are not permitted.
    • Incomplete or late submissions may result in delayed or denied approval, potentially affecting your ability to receive accreditation before the first session.
    • All application materials must be submitted together. If any documents are received less than 8 weeks before the activity start date, late fees will apply.
    • If revisions or additional information are requested, they must be submitted within 5 business days of the request. The review process will not proceed until all required updates are received. Failure to meet this deadline may result in a non-approval status.
    • Submit PowerPoint presentations with no hidden slides, as a .pdf. Accreditation is based on the submitted materials, including any required modifications. Any changes to content or format after submission must be approved in advance; unapproved changes will void the accreditation.

Note: If you are unable to complete the application in one sitting, you can save your progress by clicking the "Save" button at the end of the page. You will be prompted to create an account and will receive a link to return and complete your submission later.
