Parental Authorization and Medical Release
By checking, I parent or guardian of the child whose name is listed on this registration form hereby gives approval for his/her participation in The Boys & Girls Club of Magnolia athletics. I know that even under well supervised, normal play that injuries may occur, and do assume all risks and hazards incidental to such participation; including transportation to and from all activities. I do hereby waive, release, absolve, and indemnify and agree to hold harmless The Boys & Girls Club of Magnolia, Inc., its board of directors and officers, its staff, the league organizers, sponsors, supervisors, participants, and persons transporting the child to and from activities, for any claim arising out of injury of the child, except to the extent and in the amount covered by accident and/or liability insurance held by The Boys & Girls Club of Magnolia, Inc. I also grant permission to managing and or coaching personnel or other league representatives to authorize and obtain medical aid and treatment from any licensed physician, hospital, or medical clinic including major surgery, deemed necessary by a duly licensed physician, should the child become ill or injured while participating in Boys & Girls Club activities, at times when neither parent/guardian is available to grant authorization for emergency treatment. The largest area of growth in tobacco use is among small children. In an effort to address this problem The Boys & Girls Club of Magnolia is a tobacco free zone. So, since the 1996 baseball/softball season smoking, dipping, or chewing tobacco has not been allowed on the grounds. This includes staff, officials, coaches, players, parents, and spectators.*

Parent/Guardian Code of Conduct
By checking, I parent or guardian of the child whose name is listed on this registration form hereby gives approval for his/her participation in The Boys & Girls Club of Magnolia athletics. Parent/Guardian Code of Conduct Good Behavior from Parents/Guardians helps to ensure a positive sports experience for every athlete. • I will put the emotional and physical well being of youth participants ahead of a personal desire to win.• I will respect players, coaches, official, spectators, and families at all times.• I will respect the decisions of officials and/or coaches.• I will not engage in any violence or verbal threats or use profanity• I will allow the coaches to do their job and avoid coaching from the sidelines• I will model good sportsmanship for all youth participants• I will not belittle or ridicule anyone involved in a youth sporting event in public, private, in-person, or online.• I will not taunt any player, coach, parent, official, or spectator.• I will do my part to keep sports fun and positive for every youth participant.Parent/Guardian behavior trickles down to the players and teams. By starting with the parent code of conduct you will help to set expectations that everyone at your organization is held to high standards. • I understand that this is a game for the youth and not the adults• I understand that violating the Parents/Guardians Code of Conduct may lead to removal from the Boys and Girls Club of Magnolia.*

Athlete Code of Conduct:• I will be a good sport to players, coaches, officials, opponents, and parents at every game and practice, no matter win or lose.• I will learn the value of commitment by participating in as many practices andgames as I can.• I will encourage my teammates and praise good efforts.• I will be honest, fair, and respectful to others at all times.• I will aim to learn all I can from sports.• I will arrive at practice on time and be ready to dedicate my attention to my coaches.*
