NOTICE: This Release Form is a Contract with Legal Consequences.
Read it Carefully Before Signing.
In consideration of (my children or guests) being permitted to engage in Family Fall Festival on Saturday, October 18, 2025, I hereby make the following contractual agreements.
I fully realize the inherent dangers associated with the Family Fall Festival, including without limitation, the risks associated with falls, sprains or any other injuries. I fully realize and assume the risks associated with the fact that Mt. Zion Assembly of God nor no member of Mt. Zion Assembly of God will be responsible for any injuries I, my children or guests will incur.
I hereby waive and release for myself (my children or guests), my heirs, executors, administrators, legal representatives, assigns, and successors in interest any and all claims which I now or hereafter may have against Mt. Zion and its affiliates, agents, officials, and members (collectively, the “Releases”) for any and all injuries and/or damages which may be sustained by me or (my children or guests) directly or indirectly because of my participation and engagement with the Family Fall Festival. I further agree to indemnify and hold harmless the Releases from and against all claims arising out of my or (my children or guests) attendance at the Family Fall Festival Event.
I agree, for myself (my children and guests) and successors, that the above representations are contractually binding and are not mere recitals.
