I, the undersigned parent/guardian, understand that participation in the Summit Park District T-Ball Program involves physical activities that may include, but are not limited to, running, throwing, catching, and batting. I acknowledge that there are inherent risks of injury, including the possibility of serious injury, associated with participation. By signing below, I voluntarily assume all risks related to my child’s participation in the program.

In consideration for my child’s participation, I hereby release, discharge, and agree to hold harmless the Summit Park District, its employees, volunteers, agents, and representatives from any and all claims, demands, actions, or causes of action arising out of or in connection with participation in the program, including but not limited to injury, illness, or property damage.

I further grant permission for my child’s photograph and/or video image to be taken during program activities and used by the Summit Park District for promotional purposes.
