Thank you for your booking with Pushing Inc. Tattoo Emporium! We are excited to have you come to get a custom tattoo and are just a few steps away from finalizing your appointment.
To finalize your booking, we require a deposit; amount determined by the Artist.
You will also be required to fill out the following information to secure your appointment.
I, understand that by filling out this form I am committing to be tattooed by Pushing Inc. Tattoo Emporium
I Acknowledge that failure to contact Pushing Inc. Tattoo Emporium via PHONE 48 HOURS prior to my appointment to cancel or change will result in my deposit being null and void.
I Acknowledge that I will be notified within 24 hours if my appointment DATE will be changed.
All deposits are non refundable regardless of service.
