  •  Start of Program:

    • Once your initial payment is made and forms are completed, your first meeting (30–60 minutes) will take place. During this session, we’ll customize your workout plan, go over nutrition tips, and create injury-prevention strategies to align with your goals.

    • Your 12-week program officially begins the first Monday after your initial payment and first meeting are completed. (Note: the free 15-minute introduction does not count as the first meeting.) Please schedule your first meeting at least 2 days before the program start date.

    First Meeting (New Clients):

    • If your first meeting isn’t completed at least 2 days before the upcoming Monday start date, your program may be delayed to the following Monday.

    What’s Included:

    • A 12-week customized exercise plan on Trainerize Platform (3–6 days per week) tailored to your goals, schedule, and available equipment.
    • Injury-prevention and mobility exercises integrated into your program.
    • Weekly Google Meet video calls for progress check-ins and exercise questions.
    • In-app messaging for questions and coaching during the week (responses within 48 hours).
    • Live Coaching on nutrition (anti-inflammatory focus), recovery, and health habits during weekly meetings.
    • Active Recovery soft tissue and movement routinues (video demonstrations)

    What's Not Inlcuced:

    • Medical treatments and specific meal plans.
    • While Dr. Ellis’s experience in musculoskeletal and neurological health helps him design safe injury-prevention strategies and give general nutrition guidance, he does not provide medical treatment or prescribe specific diets.
    • Dr. Ellis is not a medical doctor, and his services do not replace professional medical advice, diagnosis, or treatment.
  • Payment Terms

  • Program Cost:

    Choose the plan that works best for you:

    • Monthly Plan: $500/mo for 12 weeks ($1,500 total)
    • In Full: $1,300

    Payment is due at the start of the program. There’s a 3-day grace period, but missed payments may pause your program until they are received. Monthly plan will automatically charge payment method on file every 4 weeks from date of initial payment. Automatic payments will cease after total payment is complete.

    Refund Policy:

    • Once the program starts, payments are non-refundable.
    • Exceptions may be made for medical emergencies (like car accidents or serious injuries) with proper documentation. In those cases, a prorated refund may be issued.

    Accepted Payment Methods:

    • We accept payments via Square: Visa, Mastercard, and American Express. Cash and check is not accepted.

    Ending the Program:

    • If Dr. Ellis ends the program:

      This may happen in cases of harassment, discrimination, or misconduct. You’ll receive written notice, and any services already provided will not be refunded. Remaining prorated amounts may be refunded at Dr. Ellis’s discretion.
    • If you choose to end the program:

      Just provide written notice. Payments already made are non-refundable, and recurring payments will continue until the total program cost is complete.
  • Post-Program Options

  • Continued Program Use:

    • Upon completion of the initial 12-week program, clients may continue to use their personalized program on the Trainerize app for $2/mo subscription, and make edits/create new workouts for themselves. Access to Trainerize app will discontinue after subscription cancellation.

    Monthly Sessions:

    • Former clients may continue to extend their coaching (monthly) with Dr. Ellis if needed.
    • 4 Week Rate: $400

    Individual Sessions:

    • Former clients may schedule individual sessions with Dr. Ellis to progress current program difficulty, accommodate for new goals, and add new injury-prevention strategies.
    • $80 per hour, payable at the time of booking.
  • Personal Training Waiver

  • I         hereby affirm that I am voluntarily starting a course of instruction in physical fitness and performance training (The Activity). I am voluntarily participating in the Activity entirely at my own risk.
  • In full consideration of the risk of injury while participating in the Activity, and for the right to participate in the Activity, I hereby, for myself, my heirs, executors, administrators, assigns, or personal representatives, knowingly and voluntarily participate in this waiver and release of liability and hereby waive any and all rights, claims or causes of action of any kind whatsoever arising out of my participation in the Activity, their affiliates, managers, members, agents, attorneys, staff, volunteers, heirs, representatives, predecessors, successors and assigns, for any kind of risks related to traveling to and from as well as participating the Activity, which may include, but are not limited to, physical or psychological injury, pain, suffering, illness disfigurement, temporary or permanent disability, economic or emotional loss, and death.

  • I acknowledge that I have carefully read this form and fully understand that it is a release of liability. I expressly agree to release and discharge the trainer or instructor from any and all claims or causes of action and I agree to voluntarily give up or waive any right that I may otherwise have to bring a legal action for personal injury or property damage. I acknowledge that I have carefully read, understood, and expressly agree to the program services and structure, post program options, and payment terms.
