The following items must be completed and emailed to NAFA® within 14 days after the tournament (late fees apply after 14 days).

□ Results must be sent electronically: Results may be emailed to scoring@flyball.org

o The host club must retain C.2 time sheets for one year. Remember to enter measuring results at https://flyball.org/tourn_publicdbaccess.shtml.

□ Jump Height Form(s) (Form C.9). Upload to NAFA using this form
□ If applicable, include any statements with respect to conduct prejudicial to the sport of NAFA Flyball. These too can be uploaded using this form.
□ If applicable, include any protest(s).
□ If applicable include Form C.15 Judges Report Form
□ No deduction from tournament fees shall be made without prior approval of the executive director.

Note that events using one NAFA EJS have a $360.00 USD minimum excluding late fees. Events using two NAFA EJS have a $1080 USD minimum.
