  • Please be detailed when completing this registration form. Having the most accurate information when caring for your children helps us ensure you and your child(ren) have the best experience possible!

    Here are a few things to know before your first visit:

    • A $45 one time family registration fee is due before your first visit.
    • Immunization records or waivers are required within 30 days.
    • Children must be 18 months old through 10 years old to attend.
    • Children in diapers are welcome!

    At least one emergency contact other than the parents is required by the State of Nebraska. 
  • Please note the following for an Emergency Contact:

    • Do not enter yourself or a secondary guardian.
    • Please do not use guardian phone numbers.
    • A working phone number is required for emergency contacts.
    • The emergency contact has permission to pick up your children and make decisions in an emergency.
    • An emergency contact is required by the State of Nebraska because we are a licensed child care center. We cannot make exceptions to this requirement.

  • Sunscreen Permission Form

    As the parent/guardian of the above child/ren, I recognize that too much exposure to UV rays may be harmful. Therefore, I give permission for the staff at The Nook to apply a sunscreen product that is broad spectrum with SPF 15 or higher, as specified below, when he/she will be playing outside, especially between the months of March through October and between the time of 10am and 4pm.

  • Medications

    Please let us know if your child(ren) takes any medications regularly. 
  • Medication Authorization

    Authorization for medications to be administered at The Nook
  • Please complete the following information with accurate and complete information. Any incomplete information may result in us not being able to administer your child's medication to them while they are in our care. Our policies regarding medications are as follows:

    • All medication must be in it's original container
    • OTC medications must be in packaging with: medication name, dosage information, and expiration date. 
    • Prescription medications must be in their original container with a prescription label that includes the child's name, prescribing doctor information, dosage time, amount, and route.

    If the medication provided to The Nook does not include all of the required information listed above we cannot give the medication to your child, no exceptions!

  • Permissions, Release, & Agreements

    Please sign and complete all agreements.
  • Bright Times, LLC (The Nook) Admission Agreement

    On behalf of myself, my spouse (if applicable), and each child I am enrolling (my “Child”), I enter into this Admission Agreement (“Agreement”) with Bright Times, LLC, a Nebraska Limited Liability Company, which is an independently owned and operated.

    Your active registration allows you to visit any participating center, as long as you have visited at least once every 12 months from your original enrollment date.

    Facility Use
    Under this Agreement, your Child may participate in drop-in, short-term child care at The Nook on a varying schedule. This includes use of our play areas and participation in activities. Full-time or reserved part-time care or private use require additional agreements. The Nook does not take field trips or provide transportation.

    Future Visits
    This Agreement, along with the Registration Form and Release, will be kept on file for at least one year past your last visit date. It will continue to apply to any future visits to any Nook center as long as you visit once every 12 months from your last visit date.

    Please note that this Agreement does not obligate The Nook to provide services indefinitely, and The Nook reserves the right to decline admission to any child at any time, without liability.

    Payment
    Payment is due at check-out for each visit. Payment for full-time care or reserved care may be required in advanced as outlined in related agreements.  Charges for drop-in care are based on the time from check-in to check-out, multiplied by the posted hourly rates. Additional posted fees may apply for services such as family registration, meals, or retail items. Unpaid balances may be subject to service fees. No refunds are provided. Any changes in fees will be posted at least 30 days in advance.

    Medical Procedures
    First Aid & Procedures: While The Nook provides a safe environment, I understand that injuries may occur. I authorize The Nook staff to provide reasonable first aid, following internal procedures, while allowing them discretion to act as circumstances require.

    Safety & Indemnity
    The Nook may take whatever action it deems appropriate to ensure the safety of my Child and other children in the facility.

    I agree to indemnify and hold harmless The Nook, its franchisees, and all associated owners, officers, directors, agents, and employees from claims, actions, or liabilities (including attorney fees and court costs) resulting from: my Child’s actions, or any inaccuracies or omissions on my Registration Form.

    Nebraska Licensing Requirements
    As required by the State of Nebraska, the licensing agency has the authority to:

    • Interview children or staff,
    • Review child and facility records,
    • Observe children’s physical condition (including signs of possible abuse, neglect, or inappropriate placement), and
    • Have children examined by a licensed medical professional.


    Additional Requirements
    As a condition of service, I confirm that I have accurately completed and signed the Registration Form and Release. The Nook will rely on this information in caring for my Child. I agree to pay all costs and attorney fees resulting from any legal action related to this Agreement, the Registration Form, or the Release.

  • Health Policy

  • At The Nook, we take steps to prevent the spread of illness:

    • Staff and children wash hands upon arrival and throughout the day.
    • Toys, equipment, and bathrooms are disinfected regularly.
    • Immunization records or valid waiver are required.
    • Parents are notified of any communicable disease outbreak.
    • We strictly enforce our Health Contract.
    • Children and staff will be excluded from care if they show signs of illness such as fever (100°F+), vomiting, diarrhea, rash with fever, severe lethargy, or other contagious symptoms. They may return after being symptom-free for 24 hours or with a doctor’s note.

    If a child becomes ill during the day, parents must pick up within 30 minutes. Staff follow the same guidelines. Temperatures are taken digitally (no rectal readings), and no fever-reducing medication is given.

  • llness Clearance & Parent Agreement
    A doctor’s note may be required to confirm that an illness is no longer contagious. Staff may suggest consulting your child’s physician. Children must be well enough to fully participate in daily activities. The director makes the final decision on exclusions.

    After surgery or medical treatment, a doctor’s note may be needed before returning.

    By enrolling at The Nook, you agree to follow our illness policy—keeping your child home when sick, picking them up within 30 minutes if they become ill, and ensuring they meet return-to-care guidelines before coming back.
