Liability Waiver and Release Agreement
In consideration of participating in the music classes offered by Fried Music, I, the undersigned parent or legal guardian of the participant(s), hereby agree to the following terms:
1. Assumption of Risk
I acknowledge and agree that participation in music classes involves certain inherent risks, including but not limited to minor injuries such as bumps, scrapes, or falls, and I assume full responsibility for any such risks, whether caused by negligence or otherwise.
2. Waiver and Release of Liability
I voluntarily waive, release, and discharge Fried Music, its owners, instructors, employees, and agents from any and all liability, claims, demands, or causes of action arising out of or related to participation in music classes or activities, including but not limited to any loss, damage, injury, or illness.
3. Medical Treatment
In the event of an emergency, I authorize Fried Music staff to secure emergency medical care for my child as deemed necessary. I understand that any costs associated with medical care will be my responsibility.
4. Photo/Video Release (Optional)
I grant permission for Fried Music to take photographs or videos of me and/or my child during classes for promotional purposes, including on social media, websites, or marketing materials.

5. Refund and Cancellation Policy
I acknowledge that tuition is non-refundable unless otherwise specified in Fried Music’s cancellation policy.
6. Agreement to Follow Rules
I agree to follow all class policies, guidelines, and instructions provided by Fried Music to ensure a safe and enjoyable experience for all participants.
