F3 Sports Programs (Future Foxes Sports)
Waiver, Release, and Consent Form
I hereby waive, release, and discharge any and all claims for damages for personal injury, death, or property damage which I may have, or which may hereafter accrue to me, as a result of participation in any and all Future Foxes Sports Programs activities, including but not limited to practices, games, clinics, camps, and/or competitions in all forms.
I fully understand that athletics is a physical and, at times, contact activity. I acknowledge that players of all skill levels may be injured while practicing or playing. As a parent/guardian, I accept these risks and consent to my child’s participation in Future Foxes Sports Programs. I further acknowledge that I have appropriate medical insurance coverage and agree to hold harmless the coaches, volunteers, staff, Future Foxes Sports Programs, and any facility used for activities from any and all liability in the event of injury or loss.
This release is intended to discharge in advance Future Foxes Sports Programs, its owners, directors, coaches, volunteers, staff, and facilities from any and all liability arising out of or connected with my participation (or my child’s participation) in these activities. I understand that participation involves inherent risks, including the risk of accidents and injury. With that understanding, I voluntarily assume all such risks and agree to indemnify and hold Future Foxes Sports Programs and all associated persons and entities harmless from any loss, liability, damage, cost, or expense that may arise as a result of my (or my child’s) participation.
I also grant permission to Future Foxes Sports Programs, its coaches, and volunteers to use my child’s name, voice, likeness, photographs, and audiovisual recordings for instruction, advertising, program websites, social media, publications, brochures, or any other lawful purpose.
Parental/Guardian Consent (Required if participant is under 18)
I hereby consent to my child’s participation in the above activities and execute this agreement, release, and waiver on their behalf. I certify that my child is physically able to participate in these activities. I further agree to indemnify and hold harmless Future Foxes Sports Programs, its coaches, volunteers, staff, and facilities from any loss, liability, damage, cost, or expense that may arise as a result of injury, death, or property damage sustained by my child during participation.
I have carefully read this agreement, waiver, and release, and fully understand its contents. I voluntarily sign this document with full knowledge of its significance.
Important Notice
Future Foxes Sports Programs is an independent organization and has no affiliation with any middle school or high school athletic programs in Westchester County, NY (or elsewhere). Participation in F3 Sports Programs does not guarantee placement on any school athletic team.
