Informed Consent and Acknowledgement
One completed form is required per ticket. This form must be signed and submitted prior to October 11, 2026, in order to be admitted to the Homecoming Dance.
The form must be completed by and signed by the parent and student regardless of age.
hereby give my consent to his/her participation at the Saint Joseph Academy Homecoming Dance on October 17, 2025 from 6:00 pm to 9:00 pm at Saint Joseph Academy. I understand that I hold Saint Joseph Academy, their officers, agents and employees, harmless from any and all liability or claims, which may arise out of or in connection with my child’s participation in this activity.
No drinking, smoking or use of drugs before, during, or after the event.
No inappropriate behavior on the dance floor.
All outside guests must have a valid picture ID to enter.
Students may arrive as early as 5:45pm but no later than 6:30pm. Attendees may not depart until 8:30pm.
Refunds will not be issued if admission is denied for any reason.
Students are responsible for the behavior of their guests (must be under 21 years of age and in at least 9th grade.) Both student and guest will be escorted out of the dance without a refund if any infractions of the school rules occur.
The administration reserves the right to refuse admittance to inappropriately dressed students or guests.
Reminder: All students who fall under the driving NJ Provisional License requirements (age 17), must make appropriate transportation arrangements to accommodate this law.
***Please note that admittance will be denied without purchase of a ticket accompanied by this form reflecting the signature of both the student and his/her parent/guardian. Any financial obligations are due to the school before any student can attend the Homecoming Dance.
