Camp CTA Registration

CAMP CTA welcomes children ages 6 months* – 16 years

*Children 6 months- 18 months must be pre-registered to ensure proper staffing levels.

Child care is available to CTA Members and Staff who are the primary caregivers of the child(ren) attending Camp CTA for children ages 6 months to 16 years. While you’re attending meetings, your children can enjoy their own Camp, managed by a national company specializing in childcare.

Camp CTA is a complete morning to early evening entertainment program with activities. *All children must be preregistered to ensure proper staffing levels. Children participate in age appropriate activities, including arts and crafts projects and active games, in a safe and educational environment.

There will be a $20 fee assessed for each day that a registered camper does not show up for camp. If the camper is not planning to attend the day he/she is registered we will need 24 hours notice not to be charged. If your child is not in Camp on the date you have registered your child(ren),your card will be charged the $20.

Parents can also bring meals for their own children to the center if desired. If CTA is providing the meals, there is a $10.00 per day per meal fee. If you order meals and your child/ren are not planning to attend we require 24 hours notice for your card not to be charged.

Saturday and Sunday, Breakfast, will be served from 8:00am-9:00am.

Saturday and Sunday, Lunch will be served 12:00pm-1:00pm.

Friday, Dinner will be served 6:30pm - 7:30pm.

REGISTRATION: To assure that your child has a place, please pre-register by Friday, November 21, 2025. We will assume your child will attend during the hours registered. If your schedule changes, we need as much notice as possible. We will accommodate you as best we can, based on availability. In accordance with COVID-19 safety requirements:

• If children are feeling sick, have tested positive for COVID-19, or are showing COVID-19 symptoms, they cannot attend the program and should notify Camp CTA/Jovie Nannies + Sitters.

Terms and Conditions

I/we, the undersigned parent(s)/guardian(s), in consideration of Jovie Nannies + Sitters (“Jovie”) providing the children’s activity programs for our child(ren)/ward(s), at the Camp CTA (“The Client”) event in Burlingame, CA from Friday, September 12, 2025 – Sunday, September 14, 2025 as designated below, do for myself/ourselves, my/our heirs, executors, administrators and assigns, hereby release and discharge Jovie and CTA, its officers, directors, employees, and agents, from any and all claims, demands, damages, actions, causes of action, or suites of any kind of nature whatsoever, which may arise from my/our child(ren)’s presence in the children’s activity programs except for gross negligence or willful misconduct on the part of Jovie’s officers, directors, employees, or agents.

Furthermore, I/we agree to indemnify and to hold Jovie and CTA harmless against loss from any and all claims, demands, damages, actions, causes of action, or suits of any kind or nature whatsoever, that may hereafter be made or brought by my/our child(ren)/ward(s) or by anyone on his/her/their behalf and I/we waive any and all rights of exemption under any federal and/or state laws against such claims. I/We authorize CTA and Jovie to use my child’s name and video or photograph at any time and in any manner in connection with its advertising, publicity, and public relations program. The video/photo may only be used by CTA or Jovie. No further claims will be made by me/us. Jovie staff is present to assure the safety and well being of all program participants. All participants are expected to respect themselves, other people and their property. CTA and Jovie are not responsible for acts caused by the willful misconduct of the youth. I/We hereby recognize and accept Jovie’s and CTA’s policies.

Children who have fever or any communicable disease will not be accepted in the Camp CTA Program.

I/We have read the above and understand this release. Furthermore, in the event of an emergency, Jovie or CTA has our permission to administer first aid or obtain emergency medical treatment in our child’s best interest. We agree to pay all expenses incurred due to an emergency involving our child. I/We agree that a fax or photocopy or electronic signature resulting from online completion of the form of my/our signature(s) on this form shall be deemed original and shall not affect the validify of this form.

*You will be required to sign a mandatory Event Care Release Waiver form on site before your child will be allowed into the program.

Confirmation

BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
