Participant Agreement
This Agreement is by and between Game Prep Baseball Academy, Inc. ("The Organization") and the Parent/Guardian of the player participating in programs managed or overseen by the Organization. The Organization is a private entity and includes its owners, directors and employees, affiliates, organizers, sponsors, representatives, agents and participants. In consideration of the details contained herein, the Parent/Guardian(s) acknowledge and agree to the following terms and conditions of this agreement;
I/We are the parent(s) and/or legal guardian(s) of the minor named as the Player and have the legal authority to execute this agreement.
I/We choose to participate in this program, approve the Player's participation in any and all activities as established by the Organization and acknowledge and agree that such activities may result in serious injury and protective equipment may not prevent all injuries to the Player.
I/We understand and acknowledge that it is our sole responsibility to maintain adequate health insurance on behalf of the player and that proof of such coverage is readily available in the event that medical attention may be required.
I/We agree to waive, release, indemnify and hold harmless Game Prep Baseball Academy, Inc., their owners, directors, employees, affiliates, organizers, sponsors, representatives, agents and participants from any and all claims, demands, liabilities, costs and expenses arising from injury to the Player, whether from negligence or for any cause.
The electronic acceptance of this Participant Agreement during Registration for this Program validates and executes all of the Terms and conditions contained herein. A copy of this agreement will be attached to your submission confirmation for your records.
